These forms can be used to request that Leyburn Library purchase for its collections a book or video/audio program or initiate a subscription to a periodical publication.
A book or multi-volume set to be added to the Leyburn or Science collections. We will automatically receive many new titles "on approval" from a select list of publishers, mostly university presses.
A subscription to an academic journal, magazine, newspaper, or other type of periodical publication.
A video program (in DVD or VHS format), an audio program (in CD or other format), or other type of "audiovisual medium."
